📋 Description

• Manage the overall care management of an assigned patient panel using nursing expertise and value-based care operations knowledge • Establish trusting and empathetic relationships with patients and families, providing clinical and emotional support • Advocate for patients and serve as a community liaison while supporting treatment-plan compliance • Assess patient and caregiver needs, including social determinants of health, and create patient-centric care plans • Coordinate interdisciplinary care to maintain continuity, manage kidney disease progression, utilization, and provider coordination • Monitor care-management plan effectiveness and modify interventions as needed • Follow evidence-based guidelines, workflow protocols, quality metrics, and regulatory and payer requirements • Collaborate with physicians, community providers, APPs, and other clinical disciplines on integrated care plans • Identify cost-effective measures supporting value-based care goals and resource utilization • Educate patients and caregivers on treatment options and support informed care decisions • Facilitate transitions of care and address barriers between care settings • Participate in clinical huddles and patient care conferences • Contribute to organizational process improvement and optimize workflows and engagement processes • Prepare reports and deliverables on program changes and developments • Collect and deliver reports on program success, patient outcomes, and patient/caregiver satisfaction • Perform other duties consistent with the role as assigned

🎯 Requirements

• Associate degree in nursing • Current RN License is required; Compact License preferred • Care management experience required • Certified Case Manager preferred • Intermediate skills with MS Office Suite, including Outlook and Teams • Able to work effectively in a primarily remote environment • Home internet must support at least 25 Mbps download and 10 Mbps upload; hardwired cable, fiber, or DSL recommended • Must work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information
