📋 Description

• Conduct telephonic outreach, primarily via auto-dialer, to members with multiple chronic conditions and financial or functional barriers • Assess member needs through data review, structured assessments, and direct conversation • Develop and manage individualized care plans • Coordinate services and connect members to appropriate clinical and community resources • Identify and resolve barriers that impede effective care and member independence • Monitor progress toward desired health outcomes and adjust care strategies as appropriate • Maintain strict adherence to compliance requirements • Navigate multiple computer systems simultaneously while engaging members in a structured environment with limited schedule flexibility

🎯 Requirements

• Must reside in Puerto Rico • Associate degree in any subject; no ADN, RN or LPN degrees • Must be bilingual in English and Spanish, including reading, writing and speaking • 2 or more years of experience in direct adult patient care • Experience in long-term care, home and community-based services, hospital/facility settings, case management, or managed care organizations • Experience working with members of a healthcare team • Experience navigating electronic software programs, including electronic medical records • Experience building relationships with internal and external customers • Minimum download speed of 25 Mbps and upload speed of 10 Mbps for home internet • Ability to work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information • Occasional travel to Humana offices for training or meetings may be required
