📋 Description

• Assess members' care needs and ensure services are provided in the least restrictive setting • Identify and resolve barriers to effective care • Plan and implement interventions to meet care needs • Coordinate services and monitor and evaluate case management plans against members' personal goals • Continuously monitor patient care and progress toward desired outcomes using assessments, data, conversations, and active care planning • Conduct telephonic and face-to-face assessments and evaluations • Remediate immediate and acute gaps in care and access • Assist enrollees with filing grievances and appeals • Explain MCO requirements, services, and benefits • Provide information and referrals to community resources • Guide members and families toward appropriate care resources • Facilitate interactions with payers, providers, interdisciplinary teams, and others involved in members' care • Visit hospitals and travel as needed

🎯 Requirements

• Bachelor's degree in social sciences, social work, human services, or a related field; OR unrestricted Licensed Practical Nurse (LPN) in Illinois with one year of experience conducting comprehensive assessments and providing formal services for the elderly; OR Licensed Professional Counselor (LPC); OR Licensed Social Worker (LSW); OR Qualified Mental Health Professional (QMHP) • Must reside in Illinois • 1+ years of experience in health care and/or case management • 1+ years of experience with Medicare and Medicaid recipients, long-term care, Home and Community Based Services, and/or managed care organizations • Experience using electronic medical records and other electronic information applications/software programs • Valid state driver's license • Personal vehicle liability insurance meeting the stated minimum limits or higher • Ability to work Monday–Friday during operating hours of 8:00 AM–7:00 PM CST, primarily 8:00 AM–5:00 PM CST, with schedule flexibility • Ability to meet remote-work internet requirements of at least 25 Mbps download and 10 Mbps upload • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information • Required TB screening if selected • Ability to travel up to 25% to visit members in hospitals within a 50-mile radius
