📋 Description

• Assess, plan, coordinate, implement, monitor, and evaluate the care needs of TRICARE beneficiaries by phone • Maximize beneficiaries’ capacity for self-care, quality of life, and desired clinical outcomes • Serve as the primary point of contact and coordinator for beneficiaries across the clinical spectrum • Coordinate and collaborate with Military Treatment Facilities, Care Management programs, physicians, staff, and providers • Develop fully integrated care plans addressing all beneficiary needs and conditions • Report to the Manager of Care Management • Work remotely and occasionally travel to Humana offices for training or meetings

🎯 Requirements

• U.S. citizenship required • Successfully receive interim approval for government security clearance (NBIS - National Background Investigation Services) • Must not currently live in Puerto Rico; Humana is not authorized to hire there for this contract • Current, valid, unrestricted license in the state of residency as a Mental Health/Behavioral Health Professional in at least one of: Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Mental Health Counselor (LMHC), Licensed Masters Social Worker (LMSW), and/or Clinical Psychologist (Ph.D.) • 3 years of clinical experience, potentially including managed care experience • Ability to obtain Certified Care Manager (CCM) certification within the first year of hire • Experience managing mental health crisis intervention • Preferred: active Certified Care Manager (CCM) designation • Preferred: experience in acute inpatient hospital case management • Preferred: experience with Medical/Behavioral Health Case Management Programs • Preferred: experience with TRICARE policies and/or the military health care delivery system • Ability to work an 8-hour shift Monday–Friday between 8:00 a.m. and 6:00 p.m. EST • During the first 4 weeks, availability for mandatory training from 8:00 a.m. to 5:00 p.m. EST • Home internet with at least 25 Mbps download and 10 Mbps upload speed • Dedicated workspace without ongoing interruptions to protect PHI/HIPAA information
