📋 Description

• Assess and evaluate members' needs and requirements in a telephonic environment • Guide members and families toward appropriate care and wellbeing resources • Employ strategies and techniques to manage members' physical, environmental, and psychosocial health issues • Identify and resolve barriers to effective care • Continuously monitor patient care and progress toward desired outcomes • Perform health assessments by phone • Develop active care plans • Educate members about chronic conditions and gaps in healthcare plans • Work with an assigned member population • Interpret guidelines, procedures, regulations, and professional standards to determine appropriate actions

🎯 Requirements

• Bachelor's degree in a health-related field • 3+ years of clinical acute care experience • Licensed Registered Nurse (RN) in Illinois with no disciplinary action • Ability to obtain Illinois RN licensure • Passion for improving consumer experiences • Working hours between 8:00 a.m. and 7:00 p.m. Central Standard Time, primarily 8:00 a.m. to 5:00 p.m. • Minimum home internet speed of 25 Mbps download and 10 Mbps upload • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information • Preferred: bilingual in English/Spanish, English/Polish, or English/Creole, with ability to speak, read, and write both languages without limitations or assistance • Preferred: Certified Case Manager (CCM) • Preferred: case management, discharge planning, and adult acute-care patient education experience • Preferred: managed care experience • Preferred: experience with Medicare and Medicaid recipients • Preferred: electronic case-note documentation and multiple computer applications/systems • Preferred: health promotion, coaching, and wellness experience • Preferred: knowledge of community health and social service agencies and community resources
