📋 Description

• Assess and evaluate members' needs and requirements to achieve or maintain optimal wellness • Meet members in their locations and assess needs and barriers • Connect members with quality services and resources to promote wellbeing and health outcomes • Administer initial and ongoing LTSS-related assessments using person-centered thinking approaches • Contact members telephonically and/or in person to establish goals and priorities • Evaluate resources, develop plans of care, and identify LTSS providers and community partnerships • Continuously modify service plans and involve applicable care team members in care planning • Support members with LTSS, environmental, social, housing, educational and other service needs • Assist members in maintaining Medicaid eligibility • Collaborate with Medical Director, Geriatrician and Care Coordinator to ensure cohesive service delivery and positive outcomes • Avoid duplicative assessments using available member information

🎯 Requirements

• Bachelor's degree in health and human services field • 2 or more years of related experience • Knowledge of Microsoft Office Word and Excel • Exceptional verbal/written communication and interpersonal skills • Ability to use a variety of electronic information applications/software programs • Required TB screening for this patient-facing role • Valid state driver's license • Personal vehicle liability insurance meeting state minimum limits or specified Humana limits, whichever is higher • Separate room with a locked door usable as a home office • Must be commutable daily to one of the covered Indiana counties: Benton, White, Fountain, Clinton, Tippecanoe, Warren, or Carroll • Prior experience with Medicare & Medicaid recipients preferred • Experience with electronic case note documentation and multiple computer applications/systems preferred • Experience with health promotion, coaching and wellness preferred • Knowledge of community health and social service agencies and additional community resources preferred • Home internet with minimum download speed of 25 Mbps and upload speed of 10 Mbps
