📋 Description
• Lead and coordinate care for complex patient populations • Practice with a high level of autonomy, including prescribing • Collaborate with physicians, care managers, and health plans • Support proactive care management and early intervention • Assist patients through transitions of care (hospital to home) • Provide virtual care and guidance to reduce unnecessary ER visits
🎯 Requirements
• Must be located in Arizona • 3–5+ years of clinical and leadership experience • Comfortable practicing independently • Experience with telehealth or remote patient management • Experience with complex populations (Medicaid, elderly, behavioral health) • Nurse Practitioner license (AZ) • Master's degree or higher • Strong communication and care coordination skills
