• Plan, develop, implement, and oversee care plans for members with identified care management needs • Assist in developing, maintaining, and monitoring processes for timely identification and assessment of high-risk and special-needs members • Serve as an interdisciplinary team member in management of identified members • Perform clinical and social determinants of health assessments • Collaborate with physicians to determine medical needs • Conduct telephone or face-to-face member interviews to identify risk factors and care management needs • Foster professional networks with service and resource providers • Initiate, develop, plan, and implement care plans promoting quality care and service • Discharge members from care management according to plan policies and procedures • Maintain care management records and ensure data integrity in health plan information systems • Collect, evaluate, and report clinical, functional, and program activity data • Identify cases for assessing the impact of care management services and assist with data collection • Educate members' medical providers regarding members' special needs • Coordinate and communicate with community support and social service systems • Reduce care costs through effective service coordination and multidisciplinary collaboration • Collaborate with multidisciplinary teams to facilitate member care and reduce barriers • Assist with developing care management policies and procedures • Communicate departmental, member, program status, and compliance issues to the supervisor/manager • Maintain patient confidentiality and follow medical information release procedures • Identify and report gaps in the service delivery system • Maintain positive working relationships with other departments
• Graduate of an Accredited School of Nursing • Current, unrestricted Registered Nurse license to practice in Maryland • 2+ years of clinical nursing experience in an acute care or community setting • 2+ years of Care Management experience • Experience assisting members with accessing community agencies and resources • Strong computer and keyboarding skills • Proficiency in Microsoft Office 365 and common computer and office hardware • Understanding of remote work tools and platforms, including Microsoft Teams, Zoom, and SharePoint • Ability to perform mathematical skills and statistical calculations accurately • Demonstrated written and verbal communication skills • Ability to conduct presentations and training before small and large groups • Knowledge of the Americans with Disabilities Act of 1990 • Strong problem-solving, organizational, and time management skills • Ability to consistently work in a fast-paced environment • Knowledge of government-sponsored managed care programs is preferred • BSN preferred • CCM preferred • Ability to follow HIPAA guidelines and maintain confidentiality of patient information
