📋 Description

• Empower members’ families to support optimal wellness • Advocate for necessary medical services across the health care continuum • Work in a telephonic managed care setting • Apply clinical knowledge and critical thinking to facilitate individualized plans of care • Assess members’ physical, functional, psychological, environmental, educational, and financial needs • Complete needs assessments and develop individualized care plans • Assign risk stratification based on medical and social complexity • Determine ongoing call frequency and continually assess the plan of care • Monitor care plans, service provision, and benefit adequacy • Interface with providers on care coordination, including appointments, transportation, and DME • Educate members about conditions, treatment plans, benefits, services, and accessing care • Monitor progress toward goals and reassess health-status changes • Refer members to community resources and facilitate access and communication among services • Participate in interdisciplinary and client rounds to identify, clarify, or prevent risks, quality issues, or care-plan variances

🎯 Requirements

• RN licensure in Texas or Compact licensure • Current, unrestricted Registered Nurse (RN) license • Three (3) or more years of clinical experience in a Level III or above NICU • CCM certification required within the timeframe specified by company policy • Experience with data entry into a database or EHR • Must be computer literate • Excellent communication skills and organizational ability • Self-motivated and willing to learn multiple tasks • Well organized and able to prioritize tasks • Accuracy in spelling and documentation • Ability to work independently and collaboratively within a team • Excellent interpersonal skills and telephone etiquette • Positive communication at all times • Commitment to excellence in customer service • College degree preferred • Experience in a Case Management role preferred • Previous experience in a maternity management program preferred
