📋 Description

• Provide virtual support to individuals and families affected by suicidal ideation and attempts • Assist with outreach, engagement, and continuous support for a cohort of members • Provide regular check-ins and personalized care • Develop and coordinate community resources addressing medical, behavioral, violence prevention, and social service needs • Assess risk and perform crisis interventions • Ensure safety through tailored safety plans • Collaborate with members to assess care options and develop and update service plans • Leverage benefits and community resources • Educate members about mental and physical health, abuse prevention, medication, and available resources • Advocate between members, families, and providers • Assist with appointment scheduling and follow-ups • Maintain accurate records and document case notes professionally and promptly

🎯 Requirements

• Bachelor’s degree in psychology, social work, or another applicable behavioral health discipline from an accredited institution • Crisis intervention training or similar certification is required • At least 2 years of experience in a related role • Preferably experience working with at-risk adolescents and young adults • Knowledge of local resources, social determinants of health, mental health, substance abuse disorders, interpersonal violence, and social issues • Experience in behavioral health or crisis intervention • Strong understanding of mental health, substance use, and social service resources • Compassionate and detail-oriented approach
