📋 Description

• Assist with outreach, engagement, and continuous support for a cohort of members • Provide regular check-ins and personalized care • Develop and coordinate community resources addressing medical, behavioral, violence prevention, and social service needs • Assess risk, perform crisis interventions, and ensure safety through tailored safety plans • Collaborate with members to assess care options and develop and update service plans • Leverage benefits and community resources • Educate members about mental and physical health, abuse prevention, medication, and available resources • Advocate between members, families, and providers • Assist in scheduling appointments and follow-ups • Ensure members attend sessions and appointments • Maintain accurate and up-to-date records of case management activities • Document case notes professionally and in a timely manner

🎯 Requirements

• Bachelor’s degree in psychology, social work, or another applicable behavioral health discipline from an accredited institution • At least 2 years of experience in a related role, preferably working with at-risk adolescents and young adults • Crisis intervention training or similar certification is required • Knowledge of local resources, social determinants of health, mental health, substance abuse disorders, interpersonal violence, and social issues • Working hours from 10:00 AM to 6:30 PM Pacific Standard Time, Monday through Friday
