• Assist providers by ensuring medication compliance, lab monitoring, medication therapy adherence, refill authorization, accurate pharmacy preferences, and medication-history accuracy • Assist patients with medication authorization and barriers to accessing medications • Communicate with patients, families, caregivers, healthcare providers, practitioners, and pharmacy personnel • Document encounters and medication information in the electronic health record (EHR) • Forward problems to pharmacists, providers, or clinic staff for resolution • Process patient onboarding • Coordinate with providers and patients to assess eligibility for financial support • Navigate the medication authorization process • Obtain and clarify patients’ home/chronic medications • Input patients’ medication lists into the EHR • Communicate medication-history discrepancies and make clarifications with patients and prescribers • Collect prescription benefit information • Direct out-of-scope patient and family inquiries to appropriate caregivers or staff • Provide customer service to patients, family members, and care team members • Document findings in the EHR to support collaboration • Adapt communication and service delivery to the specific patient population • Remain knowledgeable about applicable laws, accreditation standards, and regulatory requirements and ensure compliance
• High school diploma or equivalent • 2 years of experience as a pharmacy technician, medical intake, or healthcare employee working with medications • Medication Therapy Management Certification through the Pharmacy Technician Certification Board (PTCB) or Medication History Certificate from the American Society of Health-System Pharmacists (ASHP), obtained within 6 months • Experience with computers, software, and web-based applications • Knowledge of pharmaceutical mathematics • Effective verbal and written communication skills • Ability to present information clearly and professionally • Ability to operate standard pharmacy and office equipment • Ability to maintain compliance with applicable federal, state, and local laws, accreditation standards, and regulatory requirements • Ability to work remotely; residents of Colorado, California, Hawaii, Illinois, Maine, Maryland, Massachusetts, Minnesota, New Jersey, New York, Vermont, Washington, and Washington D.C. are not eligible for remote work
