📋 Description

• Perform initial and ongoing patient assessments, including OASIS evaluation and documentation • Develop and implement tailored treatment plans in consultation with physicians and clinicians • Adjust treatment plans as needed • Collaborate with the interdisciplinary care team to optimize patient outcomes • Educate and support patients and families/caregivers on therapy programs • Maintain accurate clinical records • Report changes in patient condition to physicians • Supervise Physical Therapy Assistants according to agency policies and state regulations • Participate in in-services, case conferences, and meetings • Identify needs for additional services and make appropriate referrals • Confirm daily visit schedules • Communicate patient progress and changes with physicians and care teams • Document all visits and progress notes on the same day of service

🎯 Requirements

• Current state-specific PT license • Graduate of an accredited physical therapy program • Valid BLS certification • Minimum one year of PT experience • Home health experience preferred • Excellent communication, organizational, and time management skills • Valid driver’s license and access to a reliable, insured vehicle • Ability to visit homes that may not be wheelchair accessible • Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques • Ability to walk, stand, stoop, and occasionally lift or turn patients • Proficiency with electronic medical records is a plus
