📋 Description

• Perform diagnosis coding of inpatient, outpatient, and physician office medical records according to Risk Adjustment and Medicare Part C guidelines • Perform intake validity checks on submitted medical records, including physician specialty, dates of service, physician/practitioner signature, and Attestation • Record process information in systems according to contract and organizational guidelines and processes • Review Senior Coder feedback to improve accuracy and quality • Enter data accurately into encoder, systems, and other required software using a personal computer, keyboard, and/or mouse • Follow established processes and procedures • Report unique record or process issues to the Project Lead, Project Manager, or Project Director • Maintain security and confidentiality of medical records and Protected Health Information (PHI) • Meet or exceed customer/company productivity and accuracy standards of a minimum 95% IRR • Meet company attendance standards • Interact appropriately with peers, coworkers, contractors, and customers • Contribute to a positive team spirit • Perform other assigned duties and projects

🎯 Requirements

• Minimum of one (1) year of experience in coding general acute hospital inpatient and outpatient and/or multi-specialty physician office medical records, excluding internships • Must be a certified coder credentialed by a recognized institution such as AAPC or AHIMA • Acceptable certifications: CPC, CCS, RHIA, RHIT • CRC certification is a plus • Experience in abstracting and ICD-9/ICD-10 coding preferred • Experience in Risk Adjustment Data Validation or CMS-HCC audits preferred • Experience performing medical record coding audits, including complex medical record abstraction • Ability to work independently and maintain a high level of concentration • Ability to consistently perform tasks with speed and accuracy • Ability to read, analyze, and interpret physician documentation • Clear and professional written and verbal communication skills • Ability to collaborate and work well in a team environment • Proficiency in Microsoft Office Suite • Flexibility and ability to plan, prioritize, and execute multiple tasks in a fast-paced environment • Ability to maintain confidentiality and integrity
