📋 Description

• Serve as caregiver to residents during scheduled work periods in long-term care • Provide resident-centered nursing care and daily living assistance under RN supervision • Provide bathing, toileting, grooming, dressing/undressing, vital-sign collection and recording, psychosocial support, and personal care • Assist with transferring, repositioning, walking, range-of-motion and passive exercises using appropriate techniques and equipment • Complete food preparation and dining tasks; assist with meals, snacks, water, and supplements • Aid providers and nursing staff with procedures as needed • Document resident interactions and outcomes related to care, behavior, activity, and dietary needs • Communicate changing resident conditions and care concerns to the charge nurse • Collect non-invasive body-fluid specimens and gather vital signs • Provide housekeeping and laundry services • Assist with and document wellness and recreational programs; encourage resident participation • Participate in departmental or organizational committees and task forces as needed • Follow infection prevention, control, and PPE policies

🎯 Requirements

• 3 months experience in Long Term Care required (6 months preferred) • Minimum of 18 years of age • Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state-certified CNA within four months of date into the position through enrollment in a training program or certification through Endorsement • CNA training and certification required and provided by facility • Colorado: QMAP certification with the Colorado Department of Public Health & Environment required, depending on facility • Satisfactorily completes yearly department-specific competency testing • Maintains a record of continuing education credits; minimum 12 hours annually • Maintains all department-specific required certifications • When applicable, certified and registered with the Unlicensed Assistive Person (UAP) Registry • Depending on facility and required responsibilities, may require Sanford Category II Driver classification, motor vehicle report, valid driver’s license, and medical qualification to perform essential driving functions • Travel required to support multiple locations within a 75-mile radius of permanent address • Minimum of 30 hours per month • One weekend shift per month

• $7/hr premium • $1.25/hour weekend shift differential • $1.25/hour evening/night shift differential (6:00 PM – 6:00 AM) • CNA training and certification required and provided by facility • Continuing education credits and yearly department-specific competency testing • Minimum of 12 hours continuing education annually
