📋 Description

• Provide whole-person 1:1 coaching to members • Drive behavior changes that positively impact clinical outcomes and overall health status • Foster positive relationships with members and provide an excellent member experience • Empower members to self-manage chronic conditions • Assess members’ skills, knowledge, barriers, and motivation to tailor education and coaching • Use a Motivational Interviewing, member-centered approach • Attend mandated meetings and complete required online or in-person training • Follow scheduling best practices to meet coaching utilization and member needs • Participate in process improvement • Mentor less-experienced coaches by offering feedback and answering questions • Support development of education handouts, videos, podcasts, and client-facing presentations • Stay current on clinical best practices and share clinical expertise across the team and company • Provide autonomous 1:1 diabetes education and coaching using CDCES knowledge and ADCES 7 Self-Care Behaviors when coaching in the Diabetes Management product • Facilitate live diabetes-focused group coaching Learning Events as needed

🎯 Requirements

• Minimum of a bachelor’s degree • Must possess and maintain certification as a Certified Diabetes Care and Education Specialist (CDCES) • Maintain required 75 CEUs in 5 years for CDCES certification • Maintain professional development and continuing education • Apply evidence-based principles to coaching practice • Review and apply research findings • Interpret and incorporate the ADCES 7 DSME/T education model • Strong knowledge of diabetes science and latest treatment options • At least 3 years of professional experience in the healthcare field • Provide quality member education and coaching • Assess member needs, barriers, skills, knowledge, and motivation • Use a Motivational Interviewing, member-centered coaching approach • Advanced degree in a health-related field is desirable • Bilingual Spanish/English is desirable • NBC-HWC certification is desirable • Must be currently authorized to work in the United States without visa sponsorship now or in the future • Teladoc Health will not sponsor or transfer employment work visas
