• Provide comprehensive care coordination for individuals transitioning between care settings • Assess member needs and develop care plans • Coordinate services across providers and community resources • Serve as a member of the State Hospital integrated care team in the State Psychiatric inpatient setting • Assess and respond to members' health and safety during states of emergency or weather advisories • Engage members in the care coordination/care management process • Educate members about available services, natural and community supports, treatment options, and diagnoses • Link members to psychological, behavioral, educational, physical, and dental evaluations • Facilitate referrals for health-related services, community resources, social support services, and community reintegration supports • Evaluate care-plan and reintegration-support outcomes
• Must meet Qualified Professional (QP) status • Fully licensed by the NC governing board regulating Human Services professions; or fully licensed RN licensed to practice in North Carolina by the NC Board of Nursing • RN pathway requires four years of full-time experience with the MH/DD/SA population served • Master's degree in a human service field plus one year of full-time MH/DD/SA experience, or qualifying supervised alcoholism and drug abuse counseling experience • Bachelor's degree in a human service field plus two years of full-time MH/DD/SA experience, or qualifying supervised alcoholism and drug abuse counseling experience • Bachelor's degree in a non-human-services field plus four years of full-time MH/DD/SA experience, or qualifying supervised alcoholism and drug abuse counseling experience • If serving LTSS members, at least two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience • Required immunizations: annual influenza vaccination, MMR, Varicella, and Tdap • Must provide proof of required immunizations before start date or within four months of start date • Must have a valid driver's license • Must reside in North Carolina in one of the listed counties: Alamance, Orange, Durham, Wake, Johnston, Person, Granville, Vance, Franklin, Warren, Nash, Halifax, Edgecombe, Wilson, Wayne, Pitt, Greene, Lenoir, Sampson, Duplin, Guilford, Randolph, Jones, Chatham, Lee, or Cumberland • Must be able to travel within a two-hour radius of the assigned Trillium region/location • All candidates must pass a drug test as a condition of employment
