📋 Description

• Coordinate housing services to support placement and long-term housing stability • Develop and implement housing plans based on individual needs and available resources • Work with landlords, providers, and community partners to secure and maintain housing arrangements • Monitor housing stability and address issues that may impact placement or retention • Facilitate access to housing resources, supports, and related services • Provide guidance on housing options, requirements, and ongoing responsibilities • Maintain documentation of housing planning and coordination activities

🎯 Requirements

• Bachelor's degree • Two (2) years of experience working with individuals and the housing systems serving people with SMI/SPMI obtaining and maintaining Permanent Supportive Housing (PSH) • Must have a valid driver’s license • Must reside within Trillium’s Mid State Region: Anson, Guilford, Montgomery, Randolph, and Richmond counties • Must be able to travel within catchment as required • Must be legally authorized to work in the United States without requiring sponsorship for employment now or in the future • All candidates must pass a drug test as a condition of employment
