• Provide education to members/recipients and stakeholder entities about Transitions to Community Living (TCL), eligibility requirements, and the RSVP process • Educate providers and internal/external stakeholders on identifying and reporting barriers • Aid in eliminating TCL barriers and create or provide training opportunities • Coordinate with referral entities to assess member/recipient needs and educate stakeholders on appropriate services and levels of care • Identify service gaps and intervene to ensure appropriate care and coordinated services • Coordinate services across the LME/MCO system and other systems, including primary care • Provide education and assistance with local hospitals and other stakeholders related to RSVP • Provide potential members/recipients with information about the TCL process during community outreach • Educate facility stakeholders when individuals do not meet TCL eligibility and provide community resources • Perform Diversion, In-Reach, Transition Process, and Post-Transition responsibilities • Collaborate with the TCL team and community partners to identify needs and provide outreach and education • Serve as a voting member of the Vaya Local Barriers Committee • Set up, manage, and facilitate community stakeholder meetings • Facilitate and coordinate quarterly ACT/CST Collaborative Meetings • Collect data concerning TCL and collaborate with teams to resolve identified needs • Document contacts and interventions accurately in administrative health records • Support monthly reporting requirements for the Department of Justice Settlement • Work independently in the community with monthly formal supervision and informal supervision as needed
• Minimum bachelor's degree in public service administration, psychology, sociology, social work, or a human service programmatic field, plus four years of directly related human services program experience; OR four-year college degree plus four years of administrative or consultative human service program experience; OR master's degree in a related human services field plus two years of supervised clinical experience; or equivalent combination of education and experience • Directly related experience includes case management, assessment and referral, supportive counseling, intervention, psychosocial therapy, and treatment planning • Experience working with children, adolescents, and adults with MHSU and IDD diagnoses • Operational knowledge of funding sources and access to existing funds, or ability to develop or present specialized funding requests • Thorough knowledge of TCL requirements, waiver and contractual requirements, and the TCLD database • Knowledge of the current Diagnostic and Statistical Manual of Mental Disorders • Knowledge of the MHSU/IDD service array provided through LME/MCO provider networks • Knowledge of Medicaid eligibility determinations • Ability to independently identify, implement, and assess system and organizational priorities related to TCL initiatives • Ability to communicate clearly and concisely verbally and in writing • Ability to make prompt independent decisions based on relevant facts and established processes • Problem-solving, negotiation, and conflict-resolution skills • Ability to organize multiple tasks and priorities and manage an assigned caseload under deadlines • Skill in Microsoft Office products, including Word, Excel, Outlook, and Forms, and Vaya Information Systems • Must reside in North Carolina or within 40 miles of the North Carolina border • Ability to perform community travel and work in various locations • Close visual acuity, walking, crouching, reaching, talking, hearing, repetitive hand/wrist/finger motion, and lifting up to 10 pounds
