📋 Description

• Provide proactive intervention and coordination of care to eligible Vaya Health members and recipients • Conduct comprehensive Health Risk Assessments covering behavioral, physical, developmental, social, and resource needs • Develop and update person-centered care plans with members and interdisciplinary care teams • Link members with appropriate services and supports across medical, behavioral health, I/DD, TBI, pharmacy, LTSS, and social-resource networks • Administer PHQ-9, GAD, CRAFT, ACES, LOCUS/CALOCUS, and other screenings within scope • Provide education, self-management strategies, referrals, and therapeutic-support linkages • Review and document medication information in Vaya’s Care Management platform • Participate in interdisciplinary care team meetings, multidisciplinary huddles, case staffings, and high-risk team meetings • Provide crisis intervention, coordination, transitional care management, and diversion support • Coordinate transitions between levels of care and help prevent institutional placement or rehospitalization • Monitor service delivery, conduct announced and unannounced visits, and identify provider non-compliance • Ensure member health and safety, report critical incidents, and escalate concerns • Verify Medicaid eligibility and coordinate related changes or deductibles • Create and maintain accurate care-management documentation and Service Authorization Requests • Collaborate with providers, Vaya departments, community stakeholders, members, families, caregivers, and natural supports • Participate in required training and perform other duties as assigned

🎯 Requirements

• Bachelor’s degree in health, psychology, sociology, social work, nursing, or another relevant human services field • Two years of experience working directly with individuals with I/DD or TBI • Two years of prior Long-Term Services and Supports and/or Home Community Based Services coordination, care delivery monitoring, and care management experience • Must meet the criteria of being a North Carolina Qualified Professional with the population served under 10A NCAC 27G .0104 • If Bachelor’s degree is in a field other than Human Services, four years of full-time accumulated experience with the population served • If Bachelor’s degree is in Nursing and incumbent is an RN, four years of full-time accumulated experience with the population served • If Master’s degree in Human Services, at least two years of full-time accumulated experience with the population served • If Bachelor’s degree in Nursing and RN, must be licensed by the North Carolina Board of Nursing to practice in North Carolina • Proficiency with Microsoft Office products, Vaya systems, care management platform, data analysis, and secondary research • Knowledge of the Diagnostic and Statistical Manual of Mental Disorders (current version) • Knowledge of MH/SU/IDD/TBI service arrays • Ability to express ideas clearly and communicate effectively • Ability to drive and sit for extended periods, including in rural areas • Ability to complete and maintain required Vaya trainings and proficiencies • Required residency in North Carolina or within 40 miles of the North Carolina border • Required to live in or near Polk, Rutherford, or Cleveland County to effectively deliver in-person contacts • Must be able to travel
