📋 Description

• Review outpatient clinical documentation and assign appropriate ICD-10-CM and CPT codes • Ensure medical-record documentation supports code assignments • Use 3M 360 Finder for coding, APC assignment, and claim-edit resolution • Confer with physicians for documentation clarification • Monitor outpatient uncoded reports to support timely coding and billing • Resolve NCCI, medical necessity, CCI, NCD, OCE, and other claims edits • Assign injection, infusion, E/M, ICD-10-CM, CPT, chargemaster, and related codes • Report documentation insufficiencies to responsible physicians • Monitor and resolve rejected accounts and eClinicalWorks error reports • Research and resolve coding, chargemaster, medical necessity, and billing conflicts • Refer complex coding issues to the coding validator or supervisor • Update patient financial accounts and perform rebilling procedures • Perform related clerical duties and maintain coding knowledge

🎯 Requirements

• High school diploma or equivalent • Successful completion of a formal coding educational program • Coding certification required from AHIMA or AAPC • Ability to read and understand outpatient clinic, ancillary, and endoscopy medical-record documentation • One to two years of experience in outpatient coding or billing • Ability to meet and maintain established quality and productivity standards • Minimum average accuracy rating of 95% • Knowledge of outpatient ICD-10-CM Official Guidelines for Coding and Reporting • Knowledge of AHA HCPCS Coding Clinics • Knowledge of NCCI and pertinent medical necessity requirements • Ability to follow Rhode Island Hospital Facility Coding Guidelines for adult patients • Knowledge of 1995 Evaluation and Management Guidelines for patients under 18 • Ability to work under stressful conditions • Ability to sit for long periods and lift at least 25 pounds
