📋 Description

• Conduct comprehensive assessments of members' behavioral health, physical health, and psychosocial needs • Develop individualized care plans that promote improved health outcomes and overall well-being • Collaborate with providers, care teams, and community resources to coordinate appropriate care • Apply clinical judgment to identify and address risk factors, barriers to care, and complex health needs • Utilize Motivational Interviewing techniques to engage members and encourage long-term behavior change • Provide crisis intervention and ensure timely access to appropriate treatment and support services • Coordinate behavioral health and substance use disorder services across the continuum of care • Leverage technology and clinical systems to document care and manage member interactions

🎯 Requirements

• Master's Degree in Social Work, Counseling, or a related Behavioral Health field • Active, unrestricted independent Behavioral Health license (LCSW, LISW, LMSW, LCPC, LPC, LMFT, LMHC, or equivalent) • 3+ years of post-master's direct clinical experience in hospital, ambulatory, outpatient, or similar behavioral health settings • 3+ years of experience treating mental health and substance use disorders • 3+ years of experience in crisis intervention and using Motivational Interviewing techniques • Strong technical proficiency and ability to document while engaging members telephonically • Managed Care or Utilization Review experience preferred • Case Management experience preferred • Discharge Planning experience preferred • Experience working with complex populations and multidisciplinary teams preferred
