📋 Description

• Build meaningful relationships with patients and families, understanding their goals, barriers, and motivators • Create personalized care plans addressing diagnoses, social determinants, and emotional well-being • Coordinate care across providers and settings to ensure safety, quality, and continuity • Educate and empower members to make informed healthcare decisions and navigate the healthcare system • Collaborate with an interdisciplinary team and advocate for whole-person health • Use data to identify opportunities and measure impact while keeping patients at the center • Maintain compliance with regulatory standards while prioritizing compassion and connection • Adapt to evolving business needs and support cross-functional initiatives • Participate in occasional travel of up to 10% as required

🎯 Requirements

• Active Illinois RN license • Strong background in care management and complex and chronic condition management • Skilled in motivational interviewing, patient education, and patient advocacy • Ability to work independently in a remote environment while staying connected to the team • Comfort with technology and multiple EMR systems • Minimum of five (5) years of clinical experience across diverse healthcare settings • Advanced knowledge of psychiatric conditions, crisis intervention, and therapeutic communication • Ability to maintain compliance with regulatory standards • Ability to work during stated working hours and remain accessible to the team • Internet connection capable of video calls and system access, with minimum 100 Mbps download and 10 Mbps upload • Ability to protect PHI, PII, and company intellectual property
