📋 Description

• Manage and support patients’ healthcare needs via telephonic outreach • Conduct and document visits using several technology platforms and EHRs • Monitor adherence to care plans and evaluate their effectiveness • Monitor patient progress in a timely manner and facilitate changes as needed • Facilitate patient access to appropriate medical and specialty providers • Educate patients and family/caregivers about relevant community resources • Coordinate continuity of patient care with external healthcare organizations and facilities • Coordinate transitions from primary care providers to specialty care providers • Support patient self-management of disease and behavior modification interventions • Provide patient health counseling, education and instruction • Use computer applications • Make and answer phone calls for the majority of the shift • Welcome patients • Review patient medical records • Code and fill out insurance forms • Schedule appointments • Arrange hospital admissions and laboratory services • Handle correspondence, billing, and bookkeeping • Escalate patient concerns or needs to the appropriate provider • Perform other duties necessary to drive company values and fulfill the mission

🎯 Requirements

• CMA Certification • Maintaining a Certification is an ongoing requirement of this role • Minimum of 1 year work experience in a healthcare setting involving patients with complex chronic disease states preferred • Chronic Care Management experience required (CCM, PCM, APCM) • Proficiency with a personal computer • Technical and analytical skill set • Excellent communication, organizational, and interpersonal skills • Experience with Microsoft Excel preferred • Able to work remotely at home in a private HIPAA compliant workspace • Able to house company equipment needed to perform job • Broadband Internet access • Internet download speed must be at least 24 mbps and upload speed at least 4 mbps • Must successfully complete a background check, drug screening, and identity verification process prior to employment • Immigration or work visa sponsorship will not be provided • Ability to hear in normal range and wear a headset / earpiece • Good visual acuity to read computer screens, scripts, forms etc. • May sit 100% of the time when taking calls • Access to the electronic medical record (EMR) system may require the use of a personal mobile device for authentication purposes
