📋 Description

• Lead a team of care management professionals providing telephonic outreach, support, and care coordination services to Special Needs members • Manage the daily workflow and provide leadership for department staff • Oversee assessment and evaluation of members' needs and requirements to achieve an optimal wellness state • Provide managerial oversight of Humana associates in care management roles, including Care Managers and Care Coaches • Provide leadership, encouragement, and teaching of Humana Care Management techniques • Monitor and audit staff performance • Develop plans of action for process improvement • Help promote the multi-disciplinary healthcare team • Ensure compliance and accuracy regarding clinical program design, implementation, monitoring, and reporting • Work with other members of the clinical leadership team and comply with policies, procedures, performance, and reporting standards • Communicate with all levels of the organization and motivate team members to meet established goals • Lead change, promote a positive work environment, and respond to unexpected changes or demands

🎯 Requirements

• Associate's degree or higher • Active, unrestricted Registered Nurse (RN) license or Social Work (SW) license • 5 or more years of professional experience • 2 or more years of supervisory or operational leadership experience, including interim team lead, mentoring new associates, SME, project leader, etc. • Willingness to flex schedule availability, with evenings and occasional weekends • Minimum home internet speed of 25 Mbps download and 10 Mbps upload • Dedicated work space without ongoing interruptions to protect member PHI / HIPAA information • Florida applicants from a Florida work location are subject to Florida Level 2 background screening through the Care Provider Background Screening Clearinghouse
