• Provide proactive intervention and coordination of care to eligible Vaya Health members and recipients • Conduct comprehensive Health Risk Assessments covering behavioral, developmental, medical, social, and resource needs • Administer PHQ-9, GAD, CRAFT, ACES, LOCUS/CALOCUS, and other screenings within scope • Develop, update, and monitor person-centered care plans and crisis plans • Link members with formal and informal services and supports across health domains • Coordinate interdisciplinary care team meetings with members, caregivers, providers, RN, pharmacists, and other stakeholders • Support transitional care management and diversion from institutional placement • Provide crisis intervention, education, self-management strategies, referrals, and follow-up • Review provider assessments and consult with clinical staff • Monitor service provision, member health and safety, critical incidents, eligibility, and care gaps • Manage and facilitate high-risk team meetings with DSS, DJJ, CCNC, schools, and community stakeholders • Document care management activities accurately and maintain AHR compliance • Collaborate with Vaya departments and community partners to identify and address system barriers • Participate in multidisciplinary huddles, complex case staffing, trainings, projects, and other assigned duties
• Bachelor’s degree required, preferably in health, psychology, sociology, social work, nursing, or another relevant human services area • Two years of experience working directly with individuals with behavioral health conditions • Two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience • North Carolina Qualified Professional status with the population served under 10A NCAC 27G .0104 • If Bachelor’s degree in nursing and RN, must be licensed by the North Carolina Board of Nursing to practice in North Carolina • Must maintain residency in North Carolina or within 40 miles of the North Carolina border • Ability to drive and sit for extended periods, including in rural areas • Intermediate to advanced proficiency in Microsoft Office products and Vaya systems • Knowledge of the DSM and MH/SU/IDD/TBI service array • Ability to complete and maintain required Vaya trainings and proficiencies • Ability to communicate clearly and effectively • Effective interpersonal, organizational, problem-solving, negotiation, arbitration, and conflict-resolution skills • Ability to make prompt independent decisions and adapt to changing priorities • Preferred experience working directly with individuals with I/DD or TBI • Experience and knowledge of NC Medicaid programs, Medicaid Transformation, Tailored Plans, state-funded services, and accreditation requirements preferred
