• Perform telephonic outreach to members in support of HEDIS and Stars measure gap closure and re-engagement • Provide evidence-based patient education regarding engagement and recommended health screenings • Use a trauma-informed, member-centric approach • Help members move through the continuum of care based on clinical or medical need • Make referrals to community partners as appropriate • Complete chart reviews for regulatory and quality improvement projects • Use EMR systems or PDF files to complete chart abstractions and enter data into HEDIS or other specialized software • Maintain a 95% accuracy rate in abstractions and data entry, validated through peer over-reads • Communicate collaboratively with CareOregon clinics, team members, and staff • Complete required ongoing HEDIS training and testing • Participate in other assigned projects • Perform work aligned with CareOregon’s mission, vision, values, policies, compliance requirements, and strategic goals
• Current unrestricted license as a Registered Nurse in the State of Oregon • Minimum 3 years’ experience as an RN in the healthcare industry • Knowledge of or desire to learn current clinical practices, chronic diseases, health-screening guidelines, and health maintenance • Ability to use a trauma-informed approach for member-centric support • Ability to extract clinical information from medical records, accurately document it in HEDIS or specialized software, and save supporting documentation • Critical thinking skills • Strong attention to detail • Competency with PDF, MS Outlook, Word, Excel, and EMR systems • Ability to learn and adapt to new EMR systems, software, specification changes, and deadlines • Ability to maintain strict confidentiality of protected health information • Ability to prioritize work in a remote environment • Ability to manage multiple tasks and switch tasks as needed • Ability to work effectively with diverse individuals and groups • Ability to meet regulatory deliverable deadlines under pressure • Good written and verbal communication skills • Ability to see, read, and perform repetitive finger and wrist movement for at least 6 hours per day • Ability to stand and sit for at least 3–6 hours per day • Ability to speak clearly, bend, lift, carry, push, pull, and/or pinch small objects for at least 1–3 hours per day
