• Assist with day-to-day operations of the Journey and Palliative program • Coordinate, schedule, and assign Journey or Palliative patients to providers • Conduct follow-up calls to patients and families to collect data about responses to established treatment plans • Serve as contact point, advocate, and informational resource for patients, care teams, families/caregivers, payers, and community resources • Manage referrals to appropriate agencies to help clients achieve Care Plan goals and objectives • Assist patients with healthcare-system issues and financial or social barriers, including interpreters, transportation services, and prescription assistance • Assist with medical records • Demonstrate excellence in internal and external customer service • Communicate HIPAA compliance, corporate compliance, and client confidentiality • Maintain compliance with local, state, and federal regulations • Comply with the Company’s Core Values and Core Competencies • Perform other duties as assigned by supervisor
• A minimum of 2 (two) years of experience in Palliative, Hospice, or Home Care preferred • Bachelor’s degree in a health-related field preferred • Medical records experience preferred • HCHB experience strongly preferred • Strong written and verbal communication skills • Teamwork orientation • Able to take and follow through with delegated tasks and accountability • Resourcefulness in problem solving • Proven data analysis skills with previous use of an electronic health record • Proven organizational skills and ability to prioritize work to meet the needs of the agency • Intermediate proficiency in Microsoft applications: Excel, Word, Outlook, PowerPoint • Ability to provide excellent customer service and demonstrate strong cultural competency • Ability to adhere to strict confidentiality guidelines • Must have and maintain in good standing professional license, certificate, or registration, as applicable • Successful completion of a drug screen prior to employment • Legally authorized to work in the United States
