Description
• Play a critical role in Pair Team's whole-person, interdisciplinary care model • Maintain ongoing caseload of individuals and promote engagement and achievement of health goals • Use relationship-based strategies to support members with care navigation • Identify and connect individuals to community resources • Conduct periodic outreach to ensure timely follow-up • Collaborate with team members using online platforms
Requirements
• 3+ years of general work experience • 1+ year(s) of case management experience • Strong understanding of cultural fluency • High degree of empathy • Ability to work collaboratively in a multidisciplinary team • Organizational skills • Strong technical skills and comfort with technology innovation • Reliable travel to get around the Fresno community (as needed)
