📋 Description

• Manage health and disease programs in workplace and community settings through partnerships with Managed Care/ACO • Develop programs for commercial insurer contracts • Maintain biometric data databases • Monitor and review claim dollars and track return on investment of program initiatives • Collect and analyze employee wellness and benefit data • Provide individualized health and disease management education • Collaborate with interdisciplinary teams to assess, monitor, implement, and evaluate employee health needs • Coordinate participant-centered resources and foster independence in chronic disease self-management • Enroll eligible participants and provide health and disease management to reduce or eliminate high-risk behaviors • Develop and conduct onsite wellness seminars and training courses • Develop and distribute health and disease education materials • Develop and evaluate customized participant care plans • Design and implement employee health education programs • Maintain current health education materials based on healthcare technology changes • Maintain database security, confidentiality, and integrity • Assist in evaluating the effectiveness of existing programs

🎯 Requirements

• Bachelor’s degree in Nursing, Nutrition, or Dietetics • Current Illinois RN License, or Registered Dietician • 5 years of technical experience • Knowledge of Joint Commission standards, CMS Core Measure requirements, and other best practices • Ability to collect and analyze workplace employee wellness and benefit data • Ability to interact with third party payors, physicians, community agencies, and co-workers • Ability to provide individualized health and disease management education • Ability to assess, monitor, implement, and evaluate employee health needs • Ability to maintain security, confidentiality, and integrity of participant databases • Ability to meet participant scheduling requirements
